This questionnaire asks you to provide information about yourself, including your educational and training background. This survey will be used to provide information to the Basic Constable Training program and for program evaluation purposes.

The information is requested on a voluntary basis and its collection is authorized under section 38(2) of the Freedom of Information and Protection of Privacy Act.

You are not required to provide answers to any questions that you do not wish to provide. Your OPC student identification number will be used to identify responses  and link to your final grades in the program.

All personal information will be kept confidential within the Research Unit.

As well, no information will be provided in research reports that would permit identification of individuals, nor will individual information be released to other agencies, including police services.
